MWS SENIOR CARE CENTRE – EUNOS

MWS Senior Care Centre – Eunos offers seniors a safe and supportive care environment during the day, allowing their caregivers to carry on with their daily responsibilities without worries. Co-located with MWS Nursing Home – Eunos and SingHealth Polyclinic (Eunos), the Centre is part of the integrated healthcare we offer to seniors who need long-time residential care in the Eunos region.

Our trained therapists also offer rehabilitation services to people of all ages who need help regaining their strength, mobility and activity participation after an injury or illness, so that they can remain active at home and in the community.

Maintenance Day Care

For frail and/or disabled seniors who need day care services. We assist with daily living activities, meals, and activities to keep our seniors engaged.

Dementia Day Care

For people with mild to advanced dementia who may display symptoms of dementia and need day care services. The focus of our care is on stimulating cognitive abilities and slowing the progression of dementia.
Community Rehabilitation

Community Rehabilitation

Community Rehabilitation

For people with medical conditions arising from illness or injury, who require rehabilitation, such as physiotherapy and occupational therapy, to complement or complete their medical care and help them remain active in the community. Priority will be given to seniors aged 60 and above.

OUR APPROACH

Our seniors and their caregivers are at the centre of decisions regarding their care, and we work alongside them to ensure their needs are met holistically.

All our clients have a person-centred, customised outcome-based programme that gives them access to the right level of rehabilitative care in a timely manner.

How to enrol

Enrolments (subsidised by the Ministry of Health) to our programmes are done through the Agency for Integrated Care (AIC). You will need to get a referral from a hospital, polyclinic or medical social worker who is familiar with your loved ones’ condition and needs. Alternatively, you may contact AIC directly at tel: 1800-650-6060 or email enquiries@aic.sg for a referral.

We will call you upon receiving the referral to set up an appointment for an initial assessment. We will then recommend either a day care plan or a rehab care plan, depending on your needs and goals.

Our Centre provides a warm, welcoming atmosphere and a spirit of community for all our seniors. Here is a list of what you can expect:

  • Creative and social activities, such as outings, birthday celebrations and intergenerational interactions
  • Physical games and adaptive sports, such as balloon badminton
  • Cognitive stimulation, such as bingo and newspaper reading
  • Sensory-rich reminiscence activities to support memory recall
  • Nutritious meals (halal and non-halal) according to dietary needs
  • Family and caregiver support, such as respite care, sharing of resources and emotional support

Our rehabilitation services include:

  • Conducting strengthening, balance and other exercises
  • Re-learning how to dress, eat, and other daily tasks
  • Home modification, such as installing grab bars in the shower
  • Caregiver training, such as coaching caregivers on providing care

Every client will receive a customised rehab care plan, according to their needs.

Depending on the type of care required, our fee starts from $72 per day (before government subsidy and GST, and subject to change according to Agency for Integrated Care (AIC) guidelines).

Actual fees are subject to household means-testing conducted by the Ministry of Health (MOH). Singapore citizens and Permanent Residents may be eligible for government subsidies and financial assistance. The subsidies range from 20% to 75%, determined by using the National Means Testing System (NMTS).

To find out your eligibility for subsidies, approach the medical social workers at your polyclinic or hospital. They will help you initiate the application for subsidies through the NMTS. Alternatively, you may contact AIC at tel: 1800-650-6060.

Yes, we do. There will be a transportation fee. Please enquire at our Centre.

Yes, your parent’s helper is welcome to accompany them at our Senior Care Centre. In fact, it will be useful for her to observe and learn how our therapists interact with your parent so that she may apply the knowledge at home. However, do note that there may be additional charges if your helper is having her meals at the Centre or requires transport.
