MWS HOME CARE & HOME HOSPICE

Seniors who are chronically ill and need assistance with daily living activities, including those nearing the end of life, can choose to age in place at home with support by our integrated MWS Home Care & Home Hospice team.

All our patients have a customised care plan, depending on whether they have caregiving support and on the level of care required. Where appropriate, we will tap on MWS’ network of Active Ageing Centres and Nursing Homes to ensure those with chronic or life-limiting illnesses receive the best care options.

OUR APPROACH

Holistic and coordinated care

We tailor our services for patients to ensure their individual needs are met holistically, including providing basic daily care and psychosocial-spiritual support. Working with partners, e.g. hospitals and community partners, as well as with MWS Active Ageing Centres and Nursing Homes, we ensure our patients receive seamless care and coordinated management.

Nurse-led Case Management Model of Care

Every patient is assigned a primary nurse who will ensure holistic coordination of care for the patient. Our integrated team means seniors and caregivers are supported by one familiar team of doctors and nurses.

HOW TO START YOUR JOURNEY WITH US

  1. Referral from the Agency for Integrated Care (AIC)
    Ask your primary doctor to submit a referral letter to AIC.
  2. Set up an appointment
    Our staff will call you and arrange a visit. We will do an assessment of your needs.
  3. Recommend a care plan
    We will discuss with you your health goals and frequency of visits.
  4. Support your needs
    We will care for you at home and train your caregiver(s) to better support your needs.

Yes, we offer hospice care to patients who are nearing the end of life. Our hospice care focuses on pain management, symptom control, and providing emotional and spiritual support for both patients and families. We aim to enhance the quality of life and ensure comfort during this time.

Hospice care specifically focuses on patients with terminal illnesses who have a limited life expectancy. Unlike other types of home care, hospice care prioritises comfort, pain relief, and emotional and spiritual support, rather than curative treatment.

At MWS Home Care & Home Hospice, we are dedicated to providing compassionate, personalised care to patients and their families. Our team is made up of experienced healthcare professionals who specialise in palliative and home hospice care, ensuring that your loved ones receive the support they need at every stage of their illness. We focus on improving the quality of life through holistic care, including emotional, spiritual, and physical support.

We offer guidance and facilitate discussions on Advanced Care Plans (ACP) and Preferred Plans of Care (PPC) to help ensure that your loved one’s wishes are respected and followed during their hospice care journey.  

While we do not offer 24-hour live-in nurses, we provide scheduled visits and have a 24/7 hotline for emergencies and guidance.

Our 24/7 hotline is available for urgent concerns, and we will guide you on the next steps. If necessary, we can arrange additional visits as needed.

MWS provides one of the most affordable home care services in Singapore.

The actual amount payable will be based on your household’s means test, which will determine the amount of government subsidy you will be eligible for. The Agency for Integrated Care (AIC) is the government agency that conducts means testing. Inform your healthcare provider if you need subsidies. You may also enquire through AIC’s hotline: 1800-650-6060.
