Integrated Community Care Provider
As part of the Ministry of Health's (MOH) Integrated Community Care Provider (ICCP) initiative, MWS helps seniors and caregivers connect with the support they need to age well in the community. Working closely with healthcare, social service and community partners, we coordinate care, provide guidance, and connect seniors to programmes and services that support healthy, active and independent living. Whether you are looking for preventive support, care services or help navigating available resources, we are here to walk the journey with you.
Through this integrated partnership approach, we aim to enable seniors to remain active, independent and connected, while supporting them to age well and age in place.
